Consent Cover Letter for Study

Dear Participant,

I invite you to participate in a research study entitled: Black, African and Caribbean Health Community Engagement Community member perspectives on current health services and ways to improve access. The purpose of the research is to:

“Learn more about how service providers currently address the health concerns of Black, African, or Caribbean clients in the Lower Mainland. As a result, we will be able to explore gaps in the provision of health care that prevent BAC members from benefiting from the resources that are currently available. We would also like to discuss the affirming approaches service providers may have adopted. This will ultimately result in the development of practices that
better cater to these marginalized populations.”

This study is funded by the Convene Grant from Vancouver Foundation.

The enclosed questionnaire has been designed to collect information on:
your experience as a BC healthcare system user, responding to unmet health needs.

Your participation in this research project is completely voluntary. You have the option to either participate and complete the survey or decline. Your responses will be kept confidential and anonymous. if you complete and submit the survey and later decide to withdraw, we will not be able to remove your responses. Data from this research will be kept under lock and key and reported only as a collective combined total. No one other than the researchers will know your individual answers to this questionnaire.

We recognize that in answering this questionnaire, one may experience increased emotional stress. The following is a list of appropriate, available, and affordable resources and counseling services. This list will be frequently reviewed and revised for accuracy in order to safeguard the well being of our participants and be in accordance with ethical guidelines. Apart from this, there are no other known risks to participation beyond those encountered in everyday life.

- 310-6789 (no area code needed): for immediate support in times of crisis
- https://bouncebackbc.ca/ : free program delivered online or over the phone to help manage low mood, mild to moderate depression, anxiety, stress or worry of individuals 13+
- https://foundrybc.ca/virtual/ : The Foundry app provides virtual services to individuals up to 24 years old
We want to recognize and value the time and effort participants contribute to our research. After you take the survey, you will have the option of entering your name in a draw to win one of 10 $50 Amazon gift cards! If you would like to be included in the draw, we’ll ask you to provide your email address. We will not use this email address for any other purpose and will treat it as confidential. Please note that a full completion of this survey is required to be entered into this draw.

By consenting, you do not waive any rights to legal recourse in the event of research-related harm.
