INVITATION AND STUDY PURPOSE
We invite you to participate in a study about DND game experience. This is for research being conducted by graduate students at the Centre for Digital Media enrolled in a projects course: DMED 520. One of the main requirements for the course is to create a digital prototype. We are required to complete user research and testing of this prototype throughout the term.

VOLUNTARY PARTICIPATION
Your participation is voluntary. You have the right to refuse to participate in this study. If you decide to participate, you may still choose to withdraw from the study at any time without any negative consequences to yourself. To withdraw please contact the student researcher or course instructor.

Due to the confidential nature of this survey we will not be able to remove your answers once they have been submitted.

STUDY PROCEDURES
If you agree to participate, the study will consist of 10 questions and should last a total of 5 minutes:

You will be asked to complete a 5-minute survey and/or interview. This will primarily contain questions related to your experience with the app, but some may also be related to demographics (e.g. gender and age). The survey and interview will be completely anonymized, and you can opt out of any or all questions.

A student researcher will observe, take notes and provide assistance if needed. No one other than the student researchers and instructor will ever have access to the notes. Your identity will remain anonymized.

POTENTIAL RISKS OF THE STUDY
There are no foreseen risks to you by participating in this study.

We do not foresee there being anything in this study that could harm you. The theme or topic of the research and/or some of the questions we ask might upset you. Please let the student researcher know if you have any concerns.
POTENTIAL BENEFITS OF THE STUDY
By participating you will be helping students complete their course requirements and might benefit by learning about their research.

CONFIDENTIALITY
Your confidentiality will be respected. Information that discloses your identity will not be released without your consent. Your responses will remain confidential, and providing identifying information such as your name, email address or IP address is optional.

All study data (including this consent) will be identified only by a unique code number and stored with encryption and password protection in a secure platform (Centre for Digital Media’s internal server or SFU Vault). Any physical copies will be locked in a filing cabinet in a locked office at the university. No one will have access to the raw interviews or observations aside from the instructor and student researchers.

Please note that in current best practices in research, electronic data is to be preserved for future use in open access initiatives. Data is normally uploaded to an online repository and these files are stripped of any information that could identify participants (e.g., names, email addresses), to ensure confidentiality.

This study is hosted by SurveyMonkey. Any data you provide may be transmitted and stored in countries outside of Canada, as well as in Canada. It is important to remember that privacy laws vary in different countries and may not be the same as in Canada.

STUDY RESULTS
The results of this study will be used for scholarly purposes only to complete the course requirements. General statistics and trends may be shared with faculty and peers at the Centre for Digital Media. Participants will be anonymized and not be identified in any reports of the completed study.
