Introduction

BC Renal would like to ask you confidential questions in this survey to help us understand the current state of planetary health awareness and practices across our provincial renal network. In this survey, planetary health refers to reducing environmental impact while recognizing that human health and the health of our ecosystems are closely linked. Survey results will enable us to learn more about how to implement low carbon, high quality kidney care throughout the province.

If you choose to participate in this survey, you are consenting (agreeing) to participate. Completion of the survey is voluntary.

Please refrain from sharing personal, identifiable information about yourselves or others. If you are a patient, your decision to participate or not and the responses you give will not affect your care in any way.

The survey will take about 10 minutes to complete. This survey will be open until July 9th. Please complete the survey once only.

Some of the questions in this survey are about your personal views/opinions (i.e. personal information). We hope that collecting this information can lead to positive changes in kidney care.

Your personal information is protected by our privacy law in BC. This law is called the Freedom of Information and Protection of Privacy Act (FIPPA). We are collecting your information under Section 26(e) of FIPPA.

Only the project team authorized by BC Renal will access your information. Responses to this survey are confidential. When responses are reported, presented, or published, the project team will remove any information that could identify you or anyone else. Survey data will be stored securely at the Provincial Health Services Authority (PHSA) for 7 years. Survey results will be used to inform planning related to planetary health across the provincial renal network in the next year.
