Survey Information and Informed Consent
We at the BPD Society of BC strive to provide accessible, trauma-informed, culturally sensitive, evidence-based programs to support folks in building a life worth living. To help us ensure that we are providing the best services to the community, we invite you to evaluate your involvement with the Peer Support Group. We value your feedback and would appreciate if you could please take a moment to complete this form.
The following survey consists of several multiple choice and short answer questions and should not take more than 10 minutes to complete. There are no known risks associated with participating in this survey. Please ensure yourself privacy as there are sensitive questions (separate space, private room).
Your answers will be anonymous, and you do not have to identify yourself. Anonymized information may be used for educational and granting purposes. You can voluntarily withdraw consent and discontinue responding at any time, simply by exiting the browser. You may skip any question that you would prefer not to answer.
All survey responses are stored securely in SurveyMonkey’s security encrypted data centers, ensuring your privacy. Survey questions and responses will be stored only in Canada.
Additional access to your survey responses is restricted to the BPD Society of BC (Baylie McKnight, Spencer Chen, and/or another authorized representative from the BPD Society of BC) and our University of Victoria research partners (Adele Mark, Dr. Bruce Ravelli). The data will be kept for 7 years and then destroyed. Survey data is stored in SurveyMonkey’s data centers for an additional 60 days after deletion of data, after which is it permanently removed.
