BCPSLS launched its revised Safety Hazard taxonomy and report form last month to better support the identification and management of hazards. To support the launch of the revised form we have developed a quiz to help BCPSLS users test their reporting prowess.

For each question below, determine whether the scenario should be reported in PSLS as a safety hazard or a patient safety event.
