Study Title: Differences in Flow State and Flow Disposition in Jazz vs. Classical Music Practice

This study was partially funded by Simon Fraser University, through the budget available to psychology honour students to cover the distribution license of the questionnaires.
Who can participate in this study?
You are eligible to participate in this study if you are ages 16 and older, have been playing jazz or classical music for five or more years, and are recruited from music clubs at SFU and UBC.

Why are we doing this study?
We want to learn more about how flow is experienced in jazz and classical musicians, and the differences between flow as a state and as a trait.

Potential benefits of participating in this study?
In the future, others may benefit from what we learn in this study. In the event that this study is published, our findings will contribute to the scarce literature on how musicians experience flow during music practice.

Incentives?
You will also be eligible to enter a prize draw for one out of five digital gift-cards from Spotify, Steam or Netflix, each worth $10 CAD. Your email address will be used as an identifier within the prize draw. In the event that you are selected for a prize, an email notification will be sent to you.

Overall design of the study:
·       Questionnaires: You will be asked to fill out four questionnaires. The first two can be filled out immediately after this consent form. Following this, a link will be emailed to you after 12 hours, as the last two one should be filled out right after a music practice session.
·       Duration: The pre-test and post-test surveys will take about 3 minutes each to complete, and the two test questionnaires will take about 10 minutes each. In total, including the time spent reading this consent form, this study may take you about 30 minutes of your time.  

Study Results:
The results of this study will be reported in an undergraduate honours thesis and may also be published in a journal. Due to the nature of this anonymous online survey, your data will remain confidential, and will not be identifiable in any research outputs.

If you would like feedback or to receive a copy of study results, please send an email inquiry to the primary contact at lleeshi@sfu.ca.  Please note that the study results will be available around April 2022, after the completion of the data collection and analysis.

Future Use:
Data collected from this study may be used in future research directives by the primary contact. By consenting to this study, you are also giving consent for the use of this data in future research analyses.

Withdrawal from Study:
In order to withdraw, you can exit the survey at any time. Discontinuing participation or withdrawing consent in this study will not result in any penalties. Please email the primary contact for withdrawal of participant data in this case. However, after April 2022, it will not be possible to remove your data from the study, as the researcher will not be able to identify your response dataset due to removal of temporary identifiers.
Confidentiality:
Your confidentiality will be respected, and any information that discloses your identity will not be released without your consent. Only the principal investigator and primary contact will have access to the raw data, for the purposes of data analyses for the study. Participants of this study will be identified by their email addresses temporarily. The email addresses will be erased after data collection for this study has been completed, after April 2022. All data from this study will be safely encrypted and retained up to 5 years.

We advise against using an employer-issued device to complete this study, as we cannot guarantee the confidentiality of your data regarding the interception of data sent via Internet by third parties (such as your employer).
