Project AMPLIFI & Ontario eHUB - Clinician Experience Survey - Hospital - Oracle Health |
Introduction
As a staff member of a Project AMPLIFI & Ontario eHUB hospital or Long Term Care site who has implemented the solution and has treated residents from or discharged patients at either end of the Long-Term Care to Hospital, and Hospital to Hospital care transitions, you are invited to participate in the Project AMPLIFI & Ontario eHUB research survey to evaluate the effectiveness and utilization of the data integration solution.
Patients are regularly transferred between hospitals and long-term care homes to receive the most appropriate care as part of their journey through the health system. The Project AMPLIFI and Ontario eHUB study aims to review and assess implementation outcomes (utilization statistics, clinician feedback, and patient transfer data).
Answer all questions to the best of your ability. This survey should take you approximately 10 minutes to complete.
Survey data will remain anonymous. For those respondents agreeing to participate in the optional interview, all data collected will be de-identified from the contact information provided. All data will be published in aggregate form only.
By clicking the link and completing the survey, you are consenting to participate in this voluntary activity.
If you have any questions or experience any technical issues with the survey, please contact Project AMPLIFI Research Manager: sculgin@stjoes.ca
The survey has been approved by the Hamilton Integrated Research Ethics Board under Project #4406. The HiREB is responsible for ensuring that participants are informed of the risks associated with the research, and that participants are free to decide if participation is right for them. If you have any questions about your rights as a research participant, please call the Office of the Chair, Hamilton Integrated Research Ethics Board 950-521-+2100 x42013
