STUDY OVERVIEW

Study purpose
Thank you for your interest in participating in an interview for our research study called “Primary Care for Individuals with Serious Mental Illness.” The purpose of this study is to understand the experiences of  primary care providers in the prevention and management of chronic disease among people living with serious mental illness (SMI).
 
Study recruitment survey
The purpose of this short survey is to record your interest in participating in this study. To be eligible to participate, you must be a community-based family physician or primary care nurse practitioner, who is currently caring for people with serious mental illness. To be considered for an interview, you must complete the first section of the survey, including your contact information. The survey also includes a short series of questions about demographic characteristics and the nature of your current practice. It is not mandatory to answer all of these questions in order to be considered for an interview. However, the answers to these questions will be used to help us determine eligibility to participate and to choose a variety of family physicians and nurse practitioners to invite for an interview.
 
Study participation
If you are selected to participate in the study, participation will involve completing a single interview, conducted by phone or by Zoom. The interview will take no more than 60 minutes, and it will be audio recorded and transcribed. You will be asked questions relating to your experiences as a primary care provider, providing care to patients with serious mental illness. At the time of study enrolment, you will be sent a consent form to review; you will be asked to provide verbal consent to participate at the start of the interview session.
 
Recruitment survey consent
By completing the questions in the following survey, you are only consenting to us collecting the information you provide in this survey. If you are selected to participate in an interview, your survey data will be retained as part of the study’s dataset. All information provided will be kept confidential. If you are deemed ineligible to participate, your data will be destroyed. If you are eligible but not invited to participate, your survey data will be deleted once study recruitment is closed.
 
You may exit the survey at any time, and your information will not be saved.
 
The survey is administered by SurveyMonkey, a tool hosted and supported by the University of Ottawa. The survey data will be stored on University of Ottawa servers and will be accessible only to approved members of the PriSMI research team. These data will be destroyed at the end of the study. This survey and the data security provisions have been approved by the University of Ottawa Research Ethics Board.
