Purpose of the Study: The purpose of this study is to document the ways that teachers work with mathematical activities, to identify the mathematical thinking and learning of teachers and students as they engage with these activities, and to determine ways to support teachers in facilitating these activities.

Participation: I will have access to Rabbit Math activities which may be used with students in my classroom. My participation in the study will consist of taking part in an anonymous survey about my experiences using these activities in my classroom.

Risks: My participation in this study will entail that I share my views on teaching and learning, which may cause me to feel vulnerable. I have received assurance that every effort will be made to minimize these risks. The research is not intended to be judgmental and my comments and opinions will be held in confidence by the research team. 

Benefits: My participation in this study may help me reflect on my teaching. Information gathered from this study will also help other teachers with the implementation of activities in their mathematics classrooms. 

Confidentiality and anonymity: I understand that any information I share will remain confidential, the contents will be used only for supporting teachers, and my identity will be protected. In reports or publications, pseudonyms will be used for participant names so that individuals will not be able to be identified. 

Conservation of data: All electronic copies of survey data will be stored on a password protected hard drive in the Pi Lab, a secure research facility at the University of Ottawa for ten years following completion of the project.  After ten years all data will be destroyed.  

Voluntary Participation: I am under no obligation to participate and if I participate, I can withdraw from the study at any time and/or refuse to answer any survey questions, without any negative consequences.

Acceptance: I understand that by completeing the survey and submitting my survey responses I agree to participate in the above research study conducted by Dr. Peter Taylor of the Department of Mathematics and Statistics at Queen’s University and Dr. Christine Suurtamm of the Faculty of Education, University of Ottawa.
