This information is being collected for the purpose of capturing necessary information to evaluate and select potential mentee, organize mentorship events, send out email communications as well as to provide updates on the Mentorship Program with the Faculty of Applied Sciences (FAS). FAS Mentorship Program related events may be photographed and/or video-recorded by SFU for archival, education, and related promotional purposes. By submitting this form, you are giving your consent to be photographed and/or video-recorded.
