When patients are seen at the VMC there are a variety of types of paperwork that are completed.

In general, new consults are provided with a discharge note that is sent to the client and the family veterinarian and if they are referred, there is referral letter that is sent to the referring veterinarian (in addition to the discharge note).

If a patient is admitted overnight by the emergency service, a transfer document is generated and sent to the family veterinarian.

We greatly appreciate your input as we try to make improvements to our processes to meet your needs, as well as for your client and patient. Your answers are kept confidential and you are welcome to skip any question you don’t want to answer.
