You are invited to participate in a research study entitled: Exploring the Knowledge, Opinions, and Perspectives of professional contact sport athletes and their medical staff regarding Cannabis Use.

With rapidly changing cannabis regulations, we want to hear from you! This survey is intended to be anonymous and has passed rigorous University of Saskatchewan Ethics Board review and requirements in its creation. The responses to the survey are automatically anonymized and aggregated in a manner that does not allow for the identification of individual respondents nor for the attribution of individual responses to a respondent. Data will be securely housed. If you are not comfortable answering any question, feel free to leave it blank and move to the next one. Purpose and Objective of the Research:

·      The purpose of this study is to obtain an understanding of the knowledge, opinions, and perspectives of both professional contact sport athletes and their medical training staff regarding Cannabis use (therapeutically and recreationally).

·      The objectives are to detect knowledge gaps and identify worthy areas for future education plans on cannabis use therapeutically, recreationally, and harm reduction strategies.

Procedures:

The survey may take you 10-15 minutes to complete.
Participation in the survey is anonymous and your responses cannot be traced back to your identity.
You can answer only those questions that you are comfortable with.
The study results will be reported in aggregate form and no names, healthcare facilities, or any other identifying information will be used.
We aim to recruit 50-100 survey participants.
Please feel free to ask any questions regarding the procedures and goals of the study or your role.

·      This study is part of a larger project funded by the NFL/NFLPA Joint Pain Management Committee.

·      No conflict of interest is noted for the researchers or sponsors.

Potential Risks:

·      There are no known or anticipated risks to you by participating in this research.

·      Nonetheless, should you become emotionally upset in the survey and need professional assistance, a referral will be made to an appropriate health professional in your local area. Alternatively, contact can be made with your family or team doctor or confidential league support systems. Confidential helplines are available in your area via telephone should you require immediate assistance. Potential Benefits:

You may receive no personal benefits from participation in this study. Findings from this study will help to better understand the knowledge, opinions, and perspectives of professional contact sport athletes and their medical training staff regarding Cannabis use so that strategies and educational material can be developed to enhance service offerings in addition to optimizing the health and well-being of athletes attempting to access cannabis care and educational information. Decision-makers within sports leagues can use our study results to allocate additional resources or modify cannabis-related services to meet the needs of athletes looking to access treatment or information involving cannabis care.
 
Compensation:

·      There is no compensation for participating in this study.

Confidentiality:

·      Participation in this study is voluntary and you are free to choose to participate in this survey or not.

·      Anonymity will be maintained and study findings will be reported in aggregate form from the survey.

·      Although the data from this research project will be published and presented at conferences, the data will be reported in aggregate form so that it will not be possible to identify individuals.

·      This survey is hosted by Survey Monkey. Your data will be stored in facilities hosted in Canada. Please see the following for more information on Survey Monkey’s Privacy Policy.
Storage of Data:  

·      Survey results, collected as electronic data, will be stored on a password-protected computer for the duration of the research process and backed up by the University of Saskatchewan servers (i.e. DataStore). At the completion of the survey, you will be redirected to a separate page that will ask if you are interested in participating in an interview about your personal experiences with Cannabis. If you are interested, you will be required to include your name, email, and phone number so that the student researcher can contact you. Your contact information will be stored separately from any survey data on a password-protected computer at the University of Saskatchewan on secure servers (i.e. DataStore). Also, contact information will be destroyed or deleted when data collection is complete and it is no longer required. Upon completion of the study, the above-mentioned data will be stored securely in the researcher’s locked office or on the researcher’s password-protected computer at on the secure servers (i.e. DataStore) for a period of, but not limited to, five years and then will be destroyed beyond recovery.

Right to Withdraw: 

·      Participation in this survey is voluntary.

·      You can decide not to participate at any time by closing your browser, or choose not to answer any questions you do not feel comfortable with. Survey responses will remain anonymous. Since the survey is anonymous, once it is submitted it cannot be removed.
