Why we’re doing the survey:
We are collecting feedback on the Pediatric Chronic Pain Primary Care Pathway (from now on called ‘the pathway’) from young people who live with pain, their caregivers (e.g. parents), healthcare providers, and people who make decisions about healthcare services.
The pathway provides links to assessment tools, treatments, and education resources. We hope this will make it easier for young people, caregivers, and healthcare providers to talk about options for chronic pain management.
What you will do for the survey:
The pathway is linked to the survey. The first 2 pages have links to assessment tools and information about treatments. The third page is a questionnaire that a person living with pain can use to talk to their healthcare providers about what’s important to them for their pain management.
The survey questions will take you about 10 minutes to complete. There are 5 questions about who you are (e.g. your age, gender) that will help us know who took part in the survey, 11 questions asking your opinion about the pathway, and 7 extra questions for Healthcare providers and health services decision makers about use of the pathway in their practice.
Potential risks and benefits of taking part:
There are no risks expected from taking part. We hope your feedback will help us improve the pathway so that it is helpful for people living with pain and their healthcare providers.
Compensation:
No payment or gifts are offered for taking part in the survey. You can choose to enter a draw for one of five, $20 gift cards, after completing the survey. Your contact information for the draw will not be linked to your survey answers. The draw for the gift card will take place on September 15th, 2022. Winners will be contacted by email. If the winner does not reply in 1 month, another participant will be chosen.
Your Privacy:
We do not collect any identifying information in the survey, so your answers will be anonymous. Your answers will be grouped with answers from other people and only shared in group form.
Storage of Survey Answers:
Survey answers are stored for 5 years on a password-protected computer in a locked office at the Saskatchewan Health Authority. After this, all copies of the survey answers will be deleted beyond recovery.
Right to withdraw from taking part:
You can refuse to answer any question. While the survey is open you can change any of your answers. You can stop taking part at any time by closing the survey.
If you finish any questions but then decide remove them, type the word “WITHDRAW” in any of the comment boxes and we will remove all of your answers. Otherwise, answers you provide up to the point that you close the survey will be kept. You do not have to give a reason for withdrawing.
How you can find out about the survey results:
Results of the survey will be summarized and distributed through the Solutions for Kids in Pain (SKIP) and Saskatchewan Pain Society.
