Thank you for taking the time to complete this survey. This survey is seeking feedback on the BFES Mental Health Access Services through Dalton Associates.

Your feedback will be used to help improve the services we offer and in understanding how to best support members of the Brampton Fire Emergency Services team.

Your responses are confidential and anonymous. Individual survey results will NOT be shared with any members of BFES. Access to individual survey results will be strictly limited to the data and evaluation team at Dalton Associates. Data in aggregate and de-identified form may be shared in reports evaluating the program.
